Community-based Residency
Auburn University provides a unique ASHP-accredited PGY-1 Community-Based Pharmacy Residency position
through Star Discount Pharmacy in Huntsville, Alabama. Star Discount Pharmacy operates six pharmacies
where they provide many advanced and innovative community pharmacy services such as specialty (both URAC
and ACHC accredited), compounding, medication therapy management (MTM), point-of-care testing,
immunizations, group education, research, and much more. The partnership with Auburn offers the resident
faculty mentorship in areas such as research, teaching, service development, and ambulatory care and
will position the resident for careers in a variety of practice settings.
Residents trained in this program will gain over 2,000 hours of advanced education and training focused
on developing patient care, practice management, and leadership skills. Additionally, residents will
have the opportunity to conduct research and present at regional and national pharmacy conferences. The
PGY-1 Community-Based Pharmacy Residency conducted by Auburn University, Auburn, Alabama.
Resident responsibilities will include:
- Providing pharmaceutical care services including, but not limited to, MTM, disease state management,
diabetes education, health and wellness screenings, point-of-care testing, compounding, specialty
medication consults, and immunizations in a community setting.
- Providing patient care and dispensing services at the various Star Pharmacy locations in Northern
Alabama.
- Precepting and mentoring students in the early and advanced experiential program.
- Initiating new advanced services in the community setting.
- Completing a residency project and presenting the project at the Southeastern Residency Conference.
The resident will receive an adjunct faculty appointment within the Department of Pharmacy Practice of
the Harrison College of Pharmacy and have instructional duties as outlined above. In addition to the
specified teaching responsibilities, the resident will have the opportunity to participate in a teaching
certificate program.

Family Health Care (FHC) is a Federally Qualified Health Center (FQHC) located in West Central Michigan.
Founded in 1967 as one for the first community health centers in the nation, we provide medical, dental,
laboratory, radiology, behavioral health care, vision, and retail pharmacy services at six main offices
and five school-based clinics. At Family Health Care, you are part of an interdisciplinary team that is
committed to delivering quality, integrated, and comprehensive health care services that are accessible

A FHC resident will be able to provide direct patient care in community and ambulatory care settings,
while working closely with medical teams, pharmacy students, and other health care professionals. The

The residency program at Creighton University Medical Center - Bergan Mercy is designed to develop a
practitioner’s expertise in the provision of pharmaceutical care that centers on development of

accountable for optimizing drug therapy and achieving the most favorable outcomes as a member of the
medical care team. The resident will provide drug information and education to patients, other
healthcare professionals and students. The resident will become proficient in developing a means of

profession. The residency is flexible and provides the resident with a wide range of experiences.
A tertiary care facility and teaching hospital located in Omaha, Nebraska, Creighton University Medical
Center - Bergan Mercy is a level I trauma, academic medical center with a teaching and research
affiliation with Creighton University. This will be the primary rotation site for the PGY1 Pharmacy
Residency Program.
The hospital’s pharmacy department is structured as a team unit with clinical services provided via
centralized, decentralized, team-based, and rounding pharmacists. Our pharmacists collaborate with
various medical services to provide care for all patients and their clinical functions include, but are
not limited to: pharmacokinetic and anticoagulation dosing/monitoring, multi-disciplinary care team
rounding, antimicrobial stewardship, IV to PO conversions, drug information, and overall medication
therapy management.
In addition, decentralized clinical pharmacy services are provided to all patient care areas, which
includes 24-hours per day/7 days per week emergency medicine and critical care clinical services.

The PGY1 Pharmacy Residency at Eastern Idaho Regional Medical Center is a 12-month program that provides
outstanding clinical practice opportunities in acute patient care settings. Pharmacy residents actively
participate in multidisciplinary rounds and interact closely with providers, internal medicine and
family medicine residents, nurses, and other healthcare professionals to improve patient outcomes.
Residents have the opportunity to collaborate with Idaho State University College of Pharmacy clinical
faculty to develop and implement a quality-improvement/research project, submit for publication, provide
didactic teaching in pharmacy program, precept APPE students, prepare for a board-certification exam,
and obtain a teaching certificate. This residency is accredited by the American Society of Health-System
Pharmacists.
The primary training site for the residency is Eastern Idaho Regional Medical Center (EIRMC), a 292-bed
acute care hospital, the largest in the region. With its average census of 200, EIRMC provides care to
patients residing in Idaho, southern Montana, western Wyoming, and the Dakotas. EIRMC’s mission

The Educational Health Center of Wyoming (EHCW) and the University of Wyoming Family Medicine Residency
Program jointly sponsor a PGY1 pharmacy residency located in the University of Wyoming Family Practice
Clinic in beautiful Casper, Wyoming. The pharmacy residency is a full-time, one-year program starting
July 1.
The resident will gain experience providing a wide range of pharmacotherapy and educational services to
patients, providers, and staff members in a primary care/family medicine setting. Learning experiences
occur primarily longitudinally, with the exception of three introductory blocked rotations at the
beginning of the residency year. The program prepares graduates for success in a variety of career
paths, including PGY2 pharmacy residency training, employment in family medicine, ambulatory care or
general pharmacy practice, and eligibility for board certification. Residents will be delegated clinical
responsibilities under the preceptorship of an experienced pharmacist to guide the development of their
practice skills.

The Veterans Health Care System of the Ozarks has two PGY1 Pharmacy Resident positions available for the 2025-2026 residency year at our Fayetteville campus. Our facility serves approximately 57,000 Veterans through our main campus in Fayetteville and our seven Community-Based Outpatient Clinics (CBOCs) located in Arkansas (Fort Smith, Ozark and Harrison), Oklahoma (Jay) and Missouri (Branson, Joplin and Springfield). The PGY1 Community-Based Pharmacy Residency at South Dakota State University began as a joint partnership
with Liebe Drug in Milbank, SD in 2012. Since then, Liebe Drug was purchased by Lewis Drug and the
residency continued in Milbank. In 2019, the program transitioned to a Lewis Drug location in Sioux
Falls, SD. Past graduates have gone on to faculty positions, PGY-2 ambulatory care residencies,
positions with Lewis Drug and other community pharmacies, and ambulatory care positions with health
systems. The residency is centered on patient care experiences equally split between a family medicine
ambulatory care clinic with a Lewis pharmacist and a Lewis Drug community pharmacy. Residents also have
significant teaching opportunities within the college. The program has two residency positions.

Stormont Vail Hospital, a 586-bed acute care hospital in Topeka, KS. The organization has received

The Stormont Vail Health PGY1 Pharmacy Residency Program has four resident positions available. Each
resident is paired with their own Resident Coordinator who will provide support and supervision
throughout the residency training program. The program offers reimbursement for professional
organization membership, professional meeting attendance, and a professional certificate program, in
addition to employee benefits of insurance, sick leave, and paid vacation.

health provider in the greater Seattle area. Swedish is affiliated with Providence Health and Services.
We offer six (6) PGY1 pharmacy resident positions. Residents will complete core rotations in
Surgical/Medical ICU, General Medicine, Solid Organ Transplant, Outpatient Oncology, Ambulatory Care,
Administration, and Antimicrobial Stewardship with the option of completing three (3) elective rotations
to meet the resident's specific interests and needs including off-site rotations. Residents will work
closely with our physicians to provide decentralized clinical services that includes prescribing
medications under multiple collaborative drug therapy agreement protocols and rounding with the
multidisciplinary team including Neonatal/Pediatric ICU, Transplant, Family Medicine, and all adult
ICUs. Residents are required to complete a self-selected longitudinal research project that will be
presented at a regional pharmacy conference and summarized in a manuscript. Residents will also complete
various clinical projects such as medication use evaluations and drug class reviews. Residents are
required to teach either at University of Washington School of Pharmacy or Seattle University Nurse
Practitioner Program and may select to complete the optional teaching certificate program. Residents
have all recognized institutional holidays off and have fifteen (15) days of vacation.

practitioners who, as part of a healthcare team, will be able to provide quality care and personal
service to their patients in the community pharmacy or ambulatory care practice settings by maximizing
economical and staffing resources. This program will also create leaders within the profession who are
prepared to move the practice of pharmacy forward by providing patient-centered pharmaceutical care. The
resident will participate in medication therapy management, immunization administration, disease state
education programs, health and wellness program development, community outreach programs, and retail
pharmacy management. The resident will also rotate through several ambulatory care settings, including
an outpatient private cardiology clinic, Indian Health Service pediatric asthma and PharmCARE clinics,
and a transitions of care clinic. This program is affiliated with SWOSU College of Pharmacy, which
provides the resident with the opportunity to deliver didactic lectures, precept students on rotations,
and assist with various pharmaceutical care laboratories. Completion of a community-based research
project is required. Participation in the University of Kentucky College of Pharmacy-ASHP Teaching
Certificate for Pharmacists Program is available.
The residency program is based at a traditional Walgreens retail pharmacy, as well as a Walgreens
Specialty Pharmacy, in the Oklahoma City metro area. These two settings will offer the resident exposure
to common chronic disease states and complex therapies for various conditions including HIV, GI,
pulmonology, transplant, neurological disorders, and chronic inflammatory diseases. The community
pharmacists at these sites work closely with medical staff, case workers, and clinical coordinators to
provide optimal drug therapy for patients. This program is uniquely designed to create leaders in
community pharmacy. Additionally, the residency’s affiliation with Southwestern Oklahoma State
University College of Pharmacy allows the resident opportunities for teaching, research, and ambulatory
care experiences.

The Queen’s Medical Center QCIPN/POB Pharmacies PGY-1 Community-Based Residency Program (CBRP) is a
single-site residency program. The intent of this program is to build upon the Doctor of Pharmacy
(PharmD) education and outcomes to develop community‐based pharmacist practitioners with diverse patient
care, leadership, and education skills who are eligible to pursue advanced training opportunities
including postgraduate year two (PGY2) residencies and professional certifications. PGY1 graduates in
community‐based practice aims to develop pharmacy leaders who are capable of improving the health of
patients within the communities they serve. The PGY-1 resident will be based in Honolulu, Hawaii at
Queen’s Medical Center Punchbowl.

- The PGY1 Community-based Pharmacy Residency Program offered by The Department of Pharmacy Practice
and Science at The University of Iowa College of Pharmacy is designed to assist graduate pharmacists
in developing specialized clinical skills in the community pharmacy setting. Established in 1997,
the residency is a collaboration of the College and innovative practice sites, and longest
continually accredited community residency in nation.
- Community Pharmacy Residents spend 2/3 of their time providing clinical patient care services in the
community pharmacy or other ambulatory care settings. Example services include: disease state
management, medication therapy management, immunizations, medication synchronization, medication
packaging, long-acting injectable medications, transitions of care and other clinical services
specific to each site.
- Residents participate in the Pharmacy Educator Preparation Program for Residents (PEPPR) to learn
teaching philosophy and techniques and can apply those skills in educating Doctor of Pharmacy
students, other health care providers, and patients.
- Residents complete a practice-based research project with the guidance of UI College of Pharmacy
Health Service Research Faculty and participate in a peer review writing group with other community
